Team Release Form?

My friend and I will be submitting soon. Do we enter one submission with both our information in it? (example: friends info in comments?) Do both of us have to get our release forms signed? If so, how would we both upload our release forms? Thanks

    just add another blank page where the other team Members sign the release form.
